1. Who we are
The D.A.Y. Foundation (“the Foundation,” “we,” “us,” or “our”) creates accessible health and wellness spaces for underserved communities and veterans. This Privacy Policy explains what information we collect through spaces4healing.com (“the Site”), how we use it, who we share it with, and the choices you have.
If you have any question about this policy or about information we hold about you, write to us at info@spaces4healing.com.
2. Information we collect
We collect only what we need to respond to you, process a gift, or run the Site.
When you request support
The support request form asks for your name, email address, city and state, the program you are interested in, and a description of how we can help. A phone number is optional. We record the date you sent the request, and our staff add notes about its status as they work on it.
When you make a donation
Donations are processed by Stripe, Inc. on Stripe’s own secure payment page. You enter your card details there, not on our Site, and we never receive or store your card number. When a gift is completed, Stripe tells us your name, your email address, the amount, the program you chose to direct it to, and a reference number for the transaction, which we keep as a record of the gift.
When you email us
If you write to us, we keep your message and your email address so that we can reply.
Collected automatically
Like most websites, the servers that host the Site record standard technical information with each request: your IP address, the type of browser and device you use, the page requested, and the time. These records are used to keep the Site running, to diagnose problems, and to protect it against abuse.
What we do not collect
We do not use analytics or advertising services, tracking pixels, or third party scripts that follow you across other websites. We do not build profiles of visitors, and we do not sell, rent, or trade personal information.
3. Health information
Please do not send us medical records, diagnoses, Social Security numbers, or urgent clinical information through this Site. The support request form is for an initial referral only, and it is not designed to receive protected health information.
The Site is not a patient portal, and submitting a request does not create a provider and patient relationship. If you need emergency help, call 911 or your local emergency service immediately.
5. How we use information
We use the information we collect to:
- review your support request and contact you about it using the details you provided;
- connect you with programs, partners, and resources that may be able to help;
- process your donation, send your receipt, and keep accurate records of gifts;
- honour the program you asked your gift to support wherever we are able to;
- answer messages you send us;
- keep the Site secure, working properly, and protected against misuse; and
- meet our legal, accounting, and reporting obligations.
We will not use your information for any purpose unrelated to these without telling you first, and we will not add you to a mailing list unless you ask to be added.
7. How long we keep information
We keep support requests for as long as we need them to help you and to keep a record of the services we have provided. We keep donation records for as long as required by law and for our accounting and tax obligations. Technical server records are kept for a short period and then deleted. When information is no longer needed, we delete it or make it anonymous.
8. How we protect information
The Site is served over an encrypted connection. Access to the administration area requires sign in and is limited to Foundation staff. Card details never reach our systems. No method of transmission or storage is perfectly secure, however, and we cannot guarantee absolute security. If we become aware of a breach that affects your information, we will notify you as the law requires.
9. Your choices and rights
You may ask us at any time to:
- tell you what personal information we hold about you;
- correct information that is inaccurate or incomplete;
- delete your information, where we are not required to keep it; or
- stop contacting you.
To make a request, write to info@spaces4healing.com. We will respond within 30 days and may need to confirm your identity before acting on it. Depending on where you live, you may have additional rights under your state’s privacy laws, and we will honour them.
10. Children
The Site is not directed to children under 13, and we do not knowingly collect personal information from them. If you believe a child has sent us information, contact us at info@spaces4healing.com and we will delete it.
11. Links to other websites
The Site may link to websites we do not control, including Stripe and partner organizations. Their privacy practices are their own, and we encourage you to read their policies.
12. Changes to this policy
We may update this policy from time to time. When we do, we will change the date at the top of this page. If a change materially affects how we use information you have already given us, we will tell you before it takes effect.
13. Contact us
Questions, requests, or concerns about your privacy can be sent to info@spaces4healing.com.
